Documents Required for the Active Duty Military Application
These are required documents for this property. Please review the list as you are preparing to submit your application.
STEP 1: Complete the MHO Application

Contact the Military Housing Office (MHO) to request a government housing application. You may reach them by phone at (425) 304-3402 or email at Everett_Housing@us.navy.mil.
Documents needed for the MHO application:
- A copy of your PCS orders.
- Page 2-DD Form 93 (dependency data sheet from your record)
You will need to complete the following forms, which will be sent to you by the MHO:
- Sex Offender Policy & Acknowledgement Disclosure Form
- Information Release Form
- DD1746: Application for Assignment to Housing
- Contact Information Form
- Intra-Station Move Form
STEP 2: Waitlist Placement and Home Offer
Once your MHO application is approved, you will be placed on a waitlist for homes that you qualify for until you are offered a home. You will receive confirmation once you are placed on the wait list. Please note that the MHO manages and controls the wait list and home offers. Constitution Park Family Housing does not have access to the waitlist.
STEP 3: Gather the Documents Needed & Submit Your HMC Application
Once the MHO has offered you a home, a Hunt representative will contact you to gather the necessary information for completing the lease. You will receive an email with a link to the Hunt Military Communities application and a list of required documentation:
- All applicants must provide proof of Assignment Date with Official Orders.
- All applicants must present their most current Leave Earning Statement (LES) to confirm current rank.
- If all dependents (spouse/kids) are not listed on orders, you must provide the following documents:
- Marriage certificate or DD1172
- Birth certificates for every person under 18
- *If all dependents are listed on your orders, no additional documents are needed.
- If you have pets, you must provide all of the following for each pet:
- Shot records
- Photo
- Microchip Number
- City of Lake Stevens Pet License
- *For Service Animals and ESA's, you must provide a doctor's letter stating which family member requires the animal.
- If you are filing the information on behalf of your active-duty spouse, you need a power of attorney (POA).
- If you are dual-military, you must provide orders and a current LES for both members. One family member must be on another's orders; if not, you will need to provide a marriage certificate or DD1172.
- Driver's License or Photo ID for every person in the household who is 18 or older.
STEP 4: Move-In Requirements
Once Hunt approves your application, you will be able to proceed with the move-in process.
At move-in, you'll need a state-issued ID for every person in the household who is 18 or older.
